Travelers can fill out the form online or print and complete a physical copy. The ministry advises submitting the form online before departure to avoid delays upon arrival in Nigeria.

“The Nigeria Health Declaration Form is mandatory for all incoming passengers. For those who cannot complete it online, paper versions will be available upon arrival,” the ministry stated on its website. “This form is part of our efforts to prevent disease spread, monitor potential infections, and control outbreaks to safeguard public health.”

The form requires travelers to provide personal, travel, health, and contact information for the 21 days prior to their arrival.

This directive follows warnings from the Africa Centre for Disease Control (CDC) about the potential declaration of a public health emergency of continental security (PHECS) due to the emergence of a new Mpox variant, Clade Ib, particularly affecting Congo. Cases of this variant have also been reported in Rwanda, Uganda, and Kenya since mid-July.

Jean Kaseya, Director-General of Africa CDC, noted on August 8 that Mpox cases in Africa have surged by 79 percent from 2022 to 2023 and by 160 percent from 2023 to 2024.
